- Calculation for water portion of this bill per 1,000 gallons of water used.

- Calculation for sanitary sewer portion of this bill per 1,000 gallons of water used.

- Basic charge for water based on the size of the water meter in house or building.

- Basic charge for sanitary sewer based on the size of the water meter in house or building.

- All current charges for this bill.

- Location of water meter.

- Name for account.

- Account number.

- Date payment is due.

- Amount of actual payment.
